2. Save Your Receipt

We will email you a receipt immediately. Keep it for your records.

3. File Arizona Form 352

4. Enter QFCO code 10053.

Be sure to include our code so your credit is properly applied.

5. Choose Your Tax Year

You have until April 15 to make a gift and apply it to the previous tax year.
